Any index in which entries are printed in 2 or 3 columns per page will almost inevitably have some breaks between the last main heading with one or more subheadings on the bottom of a right-hand (odd-numbered) page and the rest of the subheadings on top of the first column on the following left-hand (even-numbered) page. In this case it is absolutely necessary to repeat the main heading as the first line on the left-hand page, followed by the word CONTINUED (often abbreviated to CONT.) in parentheses, e.g.: tuberculosis (CONT.)
