Found: YA or children's novel, varying font sizes

i don't remember much about this book, but i think it had something to do with water or ocean or something. the main character was a girl, possibly in a mental hospital? i remember some of the book was very large font, and some of it got smaller and smaller in part(s) of the book. it's driving me crazy! please help. i read this book around 2000-2002 and not sure if it was new then or not
