I'm not sure this is from a book, but if it is I'm hoping that LT can track it down. The quote is:
Floyd could imagine a dozen things that could go wrong; it was little consolation that it was always the 13th that actually happened.
As a quality manager with responsibility for risk management, it's a quote that makes me smile. If Floyd had been involved in a thorough risk management activity, he'd not have stopped at 12 things that could go wrong. >;-)

2001 and its sequels had a Dr. Floyd as a character. I don't recall any of his other books that use that name.
