Woman marries town doctor who later dies in an accident; she is grief stricken and pregnant

I read this book between 2005-20012 I think. This is what I remember from the plot:

The setting is sometime in the early 1900 or late 1800's. A young woman has just married a doctor (an OB i think) and they don't know each other very well. She's very naive and doesn't know anything about sex or where babies come from because her mother did not prepare her. (No this isn't a sex novel). When she arrives at her new house she is marveled they have an indoor toilet and running water. Eventually she accompanies her husband on his visits and I think helps him deliver babies. Later she becomes pregnant and her husband is killed in an accident. I think she delivers her baby (a girl?) and she is having a hard time (postpartum depression) with it. I don't remember much else, but at the end I think she leaves the town and moves with her daughter. It would be a good lead in to a sequel (I think there was one).
